ONLINE TRIPS DATABASE

The online trips database allows leaders to post their own trips for publication and Web posting. Committee Chairs enter new leaders in the database, and leaders enter trips for approval by the Committee Chair and Leadership Chair.

FORMS AVAILABLE

• Volunteer Trip Release (.pdf)
  Must have sign-up sheet for all events with liability release.
• White Mountain National Forest Use Summary (.pdf)
  Required to submit AFTER leading trips to WMNF.
  
• Trip Deposit Form (.pdf)
  Required to submit a trip which requires $ down for lodging, etc.
  
• Accident/Incident Report Form (.pdf)
  Very important. Submit this form if an incident involves the use of outside agencies (police, hospital, emergency response, search and rescue, and so on). Also, file an incident report under other circumstances at your discretion. You should consider the emotional and physical welfare of any participants involved in the incident, their attitudes towards the incident, and any potential for future complaints or lawsuits. In cases involving potentially life threatening or disabling injuries or in the case of a fatality, contact AMC headquarters (see Crisis Communication Chart) as soon as the situation reasonably permits.

  This chart lists who to contact in the event of an incident or accident that meets the criteria for filing an Accident/Incident Report.
